Here is a great way to organize individual reading goals for your students. This goal sheet has a build in rubric for the teacher or student to use to track progress. It can be used during Cafe' conferencing, guided reading groups, or after administering a running record. Goal Ideas:

Rubric Bookmark - for independent reading and goal setting
Before independent reading my students pull out their laminated reading bookmarks and write their reading goal on the back with a whiteboard marker. Their goals usually look like... My reading goal is to not bother my friend. OR My reading goal is to be a 4 star reader.
They use their bookmarks while they read and it helps remind them to stay focused and increases their stamina.

Read to Self Rubric
This read-to-self rubric can be used during the Daily Five or Cafe Literacy time. It is a great way to help students self monitor their reading behaviors to build good reading stamina.
