I use this organizer for each of my 6 guided reading groups in my 5th grade class. It is divided up into Within the Text, Beyond the Text, and About the text general prompts. I write notes under each section for each child so that when I am analyzing my anecdotal notes I can pinpoint exactly where they are struggling with comprehension. There is also a section for additional notes where I put informal running record percentages and/or other notes.
