I made this syllabus for my classroom and I have never had to implement the disciplinary clause because the classroom is focused on respect for one another. In my class, we don't use these sheets for our normal textbook/vocabulary words -- instead we use this for whenever we go off on tangents. Permiso / Spanish Bathroom and Hall Pass
Do your students ask to leave class too often? Do they need help asking for permission to leave class in Spanish?
I photocopy these on colored paper and cut them out. There are 8 passes on a sheet. In my class, students receive 2 passes per quarter, but you could do more or less depending on your students.
Students hand these in to me when they need to go to the bathroom, get water, go to their locker, etc.
In my class, students who don't use their Permisos can cash them in for bonus points
