This document is three pages that outlines the procedures for your students to write reader's response letters to you based on the book they are currently reading. This assignment is incredibly flexible, which makes it easy for students to use no matter what book they are reading.

Comprehension Reading Log Grades 2-5

Phonics Skills Checklist (Year at a Glance)
I use this assessment in my guided reading binder. I write the children's names down the side and keep track of their phonics progress throughout the year by giving them a checklist when I can see evidence of their mastery in guided reading time, in their writing, or on weekly spelling tests.
