I use this checklist for data collection and student self monitoring. Prior to class, I discuss the process with the student. If I am seeing a disruptive behavior during class, I will grab a pre-printed checklist, write the behavior on the top of the sheet (for instance, blurting or getting out of seat) and place it in front of the student. This is a forensic science lab on polygraph analysis. An overview of polygraphs and the body language of lying is included as well as a crime scenario. RTI: Academic SLIP Referral Form
A template for collecting early warning information about a student's academic and attendance record for a Response to Intervention (RTI) problem-solving team. The Student Learning Inventory of Progress (SLIP) is a checklist serves as a preliminary collection of data for reviewing the student's need and assigning interventions.
