I use this checklist for data collection and student self monitoring. Prior to class, I discuss the process with the student. If I am seeing a disruptive behavior during class, I will grab a pre-printed checklist, write the behavior on the top of the sheet (for instance, blurting or getting out of seat) and place it in front of the student. Do you have students that need/ask for a break constantly? Maybe you have students that are learning how to request a break before behavior escalates. Either way, these break cards work great! I have it set up so my students can have 3 additional sensory/ brain breaks a day. Each break correlates with a different color. The first time they ask, they take a green card and circle how they are feeling. The second time they ask, they take a yellow card. The third time they ask, they take a red card.
