This is a scale of Standard Units of Distress (SDUs), where 0 indicates no anxiety (or any other emotion) and 10 indicates the most anxious the client has ever been in their life. I created this worksheet using “Pages.” Having a client draw/write examples of what that feels like at 0, 5 and 10 will help increase understanding of how to use the scale (example not client work).
