One of my favorite and most worthwhile activities!
Wonderful document for recording family facts and memories. This is a culture assignment that will be treasured for who knows how long. I hand out as extra credit, but I strongly encourage their use... where else will this information ever be captured? Students just ask questions of parents and grandparents. Even better, have them video the conversation.

GREAT FOR REMOTE LEARNING!!! This is a list of questions to guide discussion while watching the film or to be handed out to be completed while watching the film. Privilege Checklist Activity.
This is the checklist activity that is used before my "Privilege and You" questions. I always ask students to keep this private, and not to put their name on it. Student Feedback Form
Student Feedback Form - survey-style, gives students a chance to rate the class and the teacher on general environment and culture. I give this to my students at the conclusion of each term and ask for their complete honesty so that I can gauge how things are going. There's a spot for suggestions for improvement, and names are optional to encourage them to be forthright about any concerns. I teach social studies, but feel free to change the title and language to your own subject area or class.
