Behavior Data Collection / Warning Slips CHAMPS
When students break a rule in a classroom that enforces the CHAMPS system for classroom management, use these slips to help reteach behaviors and communicate with parents. This is a contract that can be used with students who are struggling with grades and achievement. There is a portion for students, parents and the teacher to complete. The teacher's portion is really small. It's something I use for kids who aren't following through on their work and need to step up and take responsibility. Behavior Contract Template - Editable

PBIS Passport
This is a great way to teach or reteach the school expectations at the beginning of the year! This passport includes 12 areas of our school where school expectations are taught. These debit cards are used as a way for students to keep up with their PBIS rewards points.
