This is a student contract that was used in elementary school beginner band, but can extend to other grades. I a monthly calendar can be used to be checked weekly for part of the grade. Also, included are calendar templates.

Missing Instrument Sheet- student involvement when instruments are at home
Students will forget their instruments at home, whether we like it or not. It is important to keep the students engaged in the music making process, even though they do not have an instrument. As you will see, the questions and spaces for responses is acceptable to the duration of the rehearsal, as well as the points needed to retain for rehearsals.

I created this simple table to keep all members of my home accountable for their music lesson practice. Family members briefly state their goal for the week and/or how often they hope to practice. I found it motivated us to continue to practice. You could use this as an award base or simply as a visual cue.

To be used in conjunction with ClassDojo or other 'point' based motivation and behaviour management.
As students show desired/favourable behaviours and/or work hard, finish work or help others they are given a 'Music Buck'. At the end of the lesson students can trade in Music Bucks for points on ClassDojo or for rewards or prizes depending on class behavioural management system.
