I use this assessment in my guided reading binder. I write the children's names down the side and keep track of their phonics progress throughout the year by giving them a checklist when I can see evidence of their mastery in guided reading time, in their writing, or on weekly spelling tests. I had never found a log that included all of the information I wanted so I went ahead and made my own!
