I use this Correction form in my math class. It is a template that requires students to rework the problems they missed on an assessment and also write a statement about what they learned from the mistake they made! I think reflecting on the mistake helps them learn from it. Also, I allow my kids to earn back points they missed on the assessment by completing this form.
