There are many options because there were many different ideas about how this could be useful -- so there is not one but five variations -- you can decide which version works best for you

This reading record has been designed to use to record decoding and reading continuum progress.
These pages should be copied back to back, with multiple copies of pages 3-4 if you want to use one record for the whole year. One double sided page has enough space record eight conferences.
I use for small guided reading groups, individual conferences and they are small enough that if I want to grab a stack to take home they don't occupy my while briefcase.

Literacy Through Play Observation Log
This Play Observation Log is designed to capture students’ language, choices, and activities during play time. The data collected here can be used to inform writing instruction.
There are two versions of the log: a one page sheet and a two page sheet. The two page sheet offers more space for note taking.
