Student Log - Classroom Interactions and Behavior
This is a simple student log for all interactions and behavior [positive and negative!]. I found myself wanting to remember funny comments students made, notes about who they worked with best, parent interactions, notes about their home life, etc. This form helps keep track of everything regarding a student, not just negative behavior. I find this particularly helpful in writing positive notes to students, as it really can include anything I'd like.

End of Year Letter to Future Self
As the school year comes to a close, this is a short, simple page that students can fill out and decorate. I recommend printing it on colorful card stock then putting it inside an envelop entitled "Do not open until X date, making it like a mini time capsule.
