Learning Center Observation Form
Use this Learning Center Observation Form when observing the use of learning centers. The domains and attributes are tied to Louisiana's teacher evaluation system, but follow Charlotte Danielson's rubric as well. Behavior Modification Cards and Tips
My behavior modification card allows a student to track daily his behavior goals by receiving either a Yes or No if he reached his or her goal. Teachers can send this card home daily to receive a signature from parents and save them as evidence that all parties are working together to re-mediate a students negative behavior.

Educational Technology Resource Evaluation Form
Use this form to help evaluate technology resources. Administrators or coaches can assign a website or piece of software/technology and have the teacher evaluate the resource using this form. Then, teachers can share what they learned. I provide copies of all completed forms at the conclusion of the sharing session. These evaluation forms make it easy to teachers to decide if the resource is of value to their classroom instruction.
