Behavior Reflection Sheet
We use this in our first grade class to help students understand the consequences of their actions. They "Stop and Reflect" in order to make a better choice the next time.
*I have recently updated the document to include a middle grade version for 3-5 with more grade appropriate wording and formatting!
After it is completed, fold in half to keep private. The student signs it, the teacher signs it, then it goes home for the parent to sign.

Terrific Daily Behavior Plan - Editable
This editable Behavior Chart can be used as a daily behavior plan for students who need a lot of positive reinforcement throughout the day for their behavior. There are two versions - one chart divides the day into 4 time periods, the other into 7 time periods. Apology Letter Template- Restorative Practices
I LOVE LOVE LOVE this tool. How often do you ask students to write an apology letter and they simple say I'm Sorry! This apology letter template helps students because they are more thoughtful about how their actions and responses to the other party. This apology letter template provides supportive responses to students while they reflect on how their actions impacted others. Color coding, numbered responses and a word bank, help the students to complete the apology letter independently.

Office Discipline Referral- Aligns with SWIS and/or PBIS Apps

This is a monthly behavior chart that you can edit. I use it with a color coordinated clothespin clip chart. Each student has a clothespin. Their pin starts at green everyday. Tardiness Reflections
This product includes a short Timeliness Plan, requiring students to quickly reflect on why they were tardy, what they missed and how to prevent tardiness in the future. The plan is followed by a longer reflection and one page article about the impact of being tardy. I had a few students that really needed some more motivation to behave! I also needed to keep documentation for my RTI folder with a few of my little guys.
