I used this one semester for an extra credit grade every week for my students who are falling behind, or had a bad week and did not want to do any assignments. The student will answer the top portion and cut it out to paste on a bulletin board in the back of the room. At progress report time, we'd go back and look at the student's "statuses". The kids really enjoyed this activity and it worked well for middle and high school.
