Daily Behavior Chart for Middle School

This is a daily behavior chart that I use for certain students that need that instant feedback. It also helps keep communication open with the parents. It's designed for middle school students that go to different teachers throughout the day. It's designed to provide the students and parents with feedback while being simple for the teacher to fill out quickly at the end of each class. It can also be used for high school students if needed.

My STAR Test Goals - Student goal setting for standardized tests

Created by

This is a fully editable .DOC file that I have used to help my students set goals for their STAR test throughout the school year. As a bonus, students can practice their measurement & data skills. I have used this in a whole group setting, making an anchor chart with good test taking practices in order to help students brainstorm for the "two things I will do to reach my goal". I have also used this leading up to their STAR test and post STAR test during individual student conferencing.

If your district has adopted the CHAMPS model for classroom transitions and expectations, then this organizer might help you simplify the parameters for middle school and high school. Too many individual posters can be confusing for kids. Use this as a single handout or blow it up into a poster. Edit the MS Word file as needed.

Math Class Procedures and Expectations

This is the packet I give to my students on the first day of school. While it is specific to me and my class (it has my name in it), it can be edited to suit any teacher. This would be very helpful to a new teacher who is trying to figure out what his/her classroom rules and expectations are going to be specifically to a math classroom.

Student Assignment Tracker with Goals and Reflection

Are your students struggling to stay on top of assignments for your class? Are you working to help students build responsibility and independence in your classroom? If so, consider adding this assignment tracker to your classroom routine. This word file gives students a place first to set assignment completion goals and action steps, then a place to chart their class assignments. Throughout the marking period, students check off their completion habits and use these results for continued goa

This is a word document for teachers using the CHAMPS model for behavior expectations. Just laminate and post the document at a literacy or math center during any activity and then use a white board marker to circle the square over the expectation for each of the CHAMPS categories. These are a great way to manage different expectations in different parts of the room, using the same system. This document is a word document, so that you can tailor the boxes to your expectations. Editable 9 Week Planner

Students need a calendar for assignments in JUST your class! This useful reference calendar will help students manage their assignments. Here's how I use it: Print double-sided on card stock (optional card stock) and 3-hole punch for storage in binder. Students write their assignments on the days they are DUE, and check off the boxes when they complete them (or teacher can initial there). After the grace period for late work (whatever that may be for you), students cross out days/weeks that the

Student Grade sheet & How to keep a Grade sheet

Created by

Before our grades were all web-based, I required all students to keep track of their own grades on a grade sheet to encourage responsibility and ownership. This is a double-sided page with an example of how to keep a grade sheet on one side, and the grade sheet itself on the other side. This only works if you use a total points system. This is a WORD doc that you can modify. I used to type in the first few assignments to give everyone a good start.

Assignment Book or Planner for Organization

This template can be used for students who benefit from a daily/weekly Assignment book or planner or agenda (whichever wording your school uses). In my district we are very technology based. However, as a special education and regular education teacher I have learned there are some students who NEED a written planner to track assignments. There is a title page, which you can decorate to your liking. There is also a parent initial column. Feel free to tweak and add a teacher signature portion

Daily Target Behaviors

I have used this behavior contract or chart to help manage the behavior of primary, elementary, and middle school students. You may choose to target all 5 behaviors for 1 point each (total of 5) per day or target some of them for 5 points per day. 100 points are possible per week. The 100 point scale seems easier for children to understand and it is simple to graph. During discussions with the student we are able to identify behaviors that can "raise the score".

This is a checklist I use for my monitored study halls. It helps the students prioritize their assignments, stay organized, and helps the students monitor their behavior and productivity during the study hall. It is helpful for the teacher to double check the checklist for accuracy, especially when students are just starting to use it. Also, I tell students that when they reach a certain number of points they have earned a free hour, candy, computer time, etc. or whatever incentive you want t
