Goal Setting Conference Form

I created this form to use at our Fall Goal Setting Conferences with parents. It is a Word document, so you may alter to fit your needs after downloading.

There are boxes for reading, writing, math, and social goals. For each subject, there is space to list skills to work on, school actions to support this goal, and home actions to support this goal. At the bottom of the document is space for the student, parent(s)/guardian(s), and teacher(s) to sign stating they will all work towards accomplishing said goals.

I find this form very useful for helping me stay on track during conferences and making sure I cover all of the students' needs with their parents/guardians in the allotted 20 minutes we have to meet. I hope you will find this useful too!
