This log promotes positive behavior and good choices, while also keeping students accountable, as well as their parents "up-to-date." You can edit the word doc version to your liking or download the PDF version. I like to place the log in a clear protective sheet and write in thin dry erase markers to save paper, then I take a picture of their daily report and message it to parents. You could also run paper copies back to back and save them for IEP meetings and parent conferences.
