On/Off-task Observation form Use to take data on kids with attention difficulties. Each page is a 10-minute observation, separated into 15 second intervals (40 intervals total for 10 minutes) and includes a line for comparison peer data. After observing, add up the the number of intervals in which the student was on and off-task, and divide by 40 (or 80 if you do two pages for 20 minutes of observation).
