One of my favorite parts of this data sheet is the "type of call" column. I'm a special education teacher, and my mental health teammates also use this data sheet to track behavior support for kiddos.

Great for helping teach students that they are in charge of their behaviors and make goals for the next week. I use these sheets once a week with the average target behaviors (data collected from their point sheets). I help them graph and then have the students journal a positive situation they had from the week before and one thing they need to continue to work on during the current week.

I wanted to give him actionable steps he could use to recognize negative thought patterns and change them. I also added Part 2. It addresses falling asleep and the anxiety people tend to feel before going to bed. This resource has helped my son, and I hope it will do the same for the children in your life. This resource is designed to giv
