Description
Are you a special education teacher or a mental health provider? Here's the perfect data sheet if you need to track behaviors and love to see it visually. It's a pre-formatted spreadsheet that tracks the day of the week, date, time, type of behavior, event details, intervention tracking, and notes.
One of my favorite parts of this data sheet is the "type of call" column. I'm a special education teacher, and my mental health teammates also use this data sheet to track behavior support for kiddos. We record if our support was needed due to a crisis call, requested by the student, or if it was a scheduled break or group time.
This spreadsheet is editable, so feel free to adjust any columns to your needs. We've also added columns to put in setting if we want to track where the behavior is most likely occurring.
The second sheet is a timeline that pulls the information from the spreadsheet to organize behavior events by date. The cells are conditionally formatted to adjust color for the "type of call" column. This allows us to track the number of crisis calls in a day and the time of day they're mostly occurring. We've used this as a time to pre-establish scheduled breaks to help reduce crisis calls.
It's so easy to copy over the timeline view or data tracking into IEPs, FBAs, or BIPs too!
