Student Report Card -- a Self-Reflection
I made this sheet as a way to hold students accountable for their learning behaviors. It's a report card for them to fill out about their effort and behaviors. The report requires students to rate themselves on 11 key learning behaviors. There's also a spot for students to write a comment about their performance. This is a nice activity to accompany teacher report cards at the end of each marking period and/or to be used as a progress report mid-quarter.

The Wellness Wheel helps provide a visual form of assessment. The client assesses how they feel they are doing in various aspects of life (social, spiritual, physical health, etc.) by coloring on a 1-5 scale. Behavior Tally Chart (On task vs. Off Task)
I use this behavior chart when I realize my student needs more support in knowing when they are or are not on task. It is a simplistic version of a behavior chart, but focusing on one behavior at a time gives the best results. I don't tally them for every instance of being on or off task, but I do for as many as I can. I don't begin this sheet without a conference with the student, and/or with the parent depending on the types of behavior I'm seeing.
