Daily Behavior Report Template
This is a template for communicating with parents on student behavior. I use different color cards in my classroom with each student starting the day on green. This worked really well to communicate with parents, and you can use it as a something the student has to fill out for accountability or you can use it as something that you fill out yourself.

Individual Behavior Record
The teacher keeps one of these for every student. Each time a strategy is implemented, the date is recorded.
