After lots of research, I created this thorough yearbook club application. I used this one for 5th - 8th grade students at my school, but it can easily be modified to fit your needs. It could really be used as-is for high school - just take out where it says 5th- 8th ;)

Preschool Survival Kit. I make this for Back to School Night. I use it as a gift for the children and also as a conversation starter for new parents.
To assemble it; print both pages and cut to desired size, have ready enough of all items on the second page for each student, have plastic baggies or paper lunch bags and ribbon.
