Daily Reading Log with Response Choices
I struggled with my students being accountable during independent reading time. I wanted to make sure that they were really focusing and thinking while they were reading. Having them fill this out at the end of each reading session has really helped. While reading they are paying much closer attention and placing sticky notes at parts where they might want to do a response. When about 3 minutes are left of Independent reading time, the students write in a quick response.

Motivational Testing Notes
Lesson those test jitters with these motivational testing notes. I send this top secret envelope home with my students containing 6 blank notes. This is one note for each state testing day. My STAR Test Goals - Student goal setting for standardized tests
This is a fully editable .DOC file that I have used to help my students set goals for their STAR test throughout the school year.
As a bonus, students can practice their measurement & data skills.
