Behavior Reflection Sheet
We use this in our first grade class to help students understand the consequences of their actions. They "Stop and Reflect" in order to make a better choice the next time.
*I have recently updated the document to include a middle grade version for 3-5 with more grade appropriate wording and formatting!

Division Fact Timed Test (MCC3. OA. 7)
Multiplication and Division are the most prominent skills in 3rd Grade math. These timed test are a great way for students to learn their facts. I start by doing the multiplication timed test ( which are easy to find online.) When students have completed through their 12's, they begin on their division facts. These test are not as easy to find online. That is why I created these.
I give students 1 minute to complete each 16 question test.

Set the Rule and then complete the Input/ Output Table. It can be used for addition, subtraction, multiplication, or division. This can be used with Input/ Output task cards or you can partly fill in the form your self to have the students figure out the rule and the missing numbers. Kid Friendly Bloom's Taxonomy Activities
My students use this to review and build deeper understanding of topics. I would typically partner them and they would have to come up with one topic related question and answer from the front side and 2 from each box on the back with the higher level questions. Giving them guide words and question stems was helpful in developing their ability to look at a topic multiple ways on several levels.

Individual Multiplication Fluency Tracker (editable)
I use this individual tracker to measure 5th grade students' multiplication fluency. Instead of recording the grades in the gradebook, I have students fill in their scores on this sheet each time they are assessed, approximately every 2 weeks. Students are motivated when they improve their scores and continue to review their multiplication facts (0-12). If a student's score drops,he/she is reminded to have a growth mindset and strive to improve on the next assessment.

Weekly Work Reflections
When you send a packet of work home (probably in a weekly folder) have your students reflect on their behavior and grades using this printable, and then give your teacher reflection. When the packet of work goes home parents can instantly receive feedback, sign the reflection form, and add any comments/questions.
