This is a facts about graphic organizer. I've used this in library on writing down information about bats, turkeys, etc. Have mostly used it for first grade but can be used for other grades too. The lines in the right box are to write a one - two informational sentence and the left box is for the students to draw a picture of what they wrote. The second page can be used for kindergarten later in the year.
