I use this Correction form in my math class. It is a template that requires students to rework the problems they missed on an assessment and also write a statement about what they learned from the mistake they made! I think reflecting on the mistake helps them learn from it. Also, I allow my kids to earn back points they missed on the assessment by completing this form.

This long-term assignment was a chance for my 6th graders to reflect on their experiences in math throughout each Quarter (but it could be used in any content). It allowed me to assess their understanding of content and also get student feedback to help me plan for future lessons. It was presented at the beginning of the Quarter and collected at the end. The students loved it! The assignment outline, rubric used and graphic organizer are all attached.
