Teal Radford
This is a reading, writing, and problem solving, and perspective-taking exercise for young adult through adult age populations. The worksheets provide real-life ADL scenarios in which the individual must consider other perspectives and provide written or spoken responses about how they would feel and what they would do. There are no right answers, only opportunities for realistic discussion.**Example from the worksheet: You best friend tells you to steal a bag of Skittles from the grocery store.

PLS-5 Template | Speech Therapy Assessment

For my first ever product (!) I decided to offer my Preschool Language Scales - Fifth Edition (PLS-5) template. It includes a test summary, score table, and summary sections. It's available as a Word document and formatted so you can quickly and easily copy and edit it in your own report. With my templates I try and solve two problems: a) writing and formatting the basics of a test can take up unnecessary time, leaving clinicians less time to write about what the results really mean; b) parents

I created this for several of my middle school and high school students who struggle with those “What/if” or “What do you do?” situations. I encourage you to read through these and pull out ones that are not necessary for your students. While playing you may reshuffle the cards and use the same scenarios again. You may need to do this depending on the number of players you have. Some variations can also include: how others think of your choice?, what should your “friends” do in that situatio

The Receptive One-Word Picture Vocabulary Test - Fourth Edition and the Expressive One-Word Picture Vocabulary Test - Fourth Edition are common speech therapy assessments used to determine a child's receptive and expressive vocabulary. They're typically used together, and the information obtained is pretty straight-forward. I made a quick-and-easy FREE speech therapy assessment for your evaluations reports. It has test summary sections, tables for results, and result summary sections. The format

It's OK to Lose - A Social Story About Losing Games

This is just an easy to cut out and staple together little social story about losing games. Read it while your student is calm and then right before you play a game and rehearse what you're going to do over and over. I had an uneven number of pages, so there's only one of them on the first page (the cover) and then the rest of them are four per page. Cut them out and then staple along the top and you're got a kid-sized social story. :)Also, this is the "flattened" version of it, which is why it'

So this is as advertised. Not everything in this is thorough or perfect - it's just a free copy of the template I made and use in my own write ups. Fair warning, I don't actually use all subtests so they're not all in there. That said, it's a Word doc so you can edit it all you like. I know some of the tests are older, but the sub-test write ups will be very similar or even identical to the current edition, so all you'll have to do is make some tweaks. I'm actually a private SLP, so to use this
