At one school I was an advisory teacher. This meet that students were suppose to spend 2 hrs a week reading. To help hold students accountable, I had the students fill out these logs. They also helped the 8th graders, who could count community service hours on these logs for graduation. You can expand these to include other homework assignments and classwork.
