This is a child care weekly attendance record sign-in/sign-out sheet. Use this form to record the children in attendance during a week of daycare. It includes lines to record their first and last name, the time they are checked in, checked out and spot for the parent/guardian to initial when they pick up their child. Also includes lines for the child care provider name, license number, dates and additional notes.

You can use this supply list as a references for what you want the parents to bring to your in-home daycare. This was the very first supply list I gave to parents when we first opened up. You can modify this and delete things like hand soap, tissues and Clorox for the kiddos that come later in the year.
