This teacher questionnaire is a great tool for teachers to fill out and give to their district occupational therapist or a member of the IEP/RTI team. Occupational Therapy Screening Request Form
This is a form school-based occupational therapists can use for screening students. I like to give this form to all of my case managers as well as any general ed teacher who requests it; the person filling it out can check off their concerns like fine motor skills, emotional regulation, difficulty with transitions, etc. There's also a section for the OT to fill out their observations and recommendations after completing the screening.
