Orton-Gillingham Tutoring Contract

I have been tutoring privately for over 25 years and I fine-tune my tutoring policies every school year and summer. This is my latest version. You will have to "make it your own." It is NOT a one size fits all product so, of course, you will have to customize it. I always send the first 2 pages to parents after the initial phone call or email so they understand my background, my services, and Orton-Gillingham. The final 2 pages are used after a schedule has been agreed upon between the parent(s)

Daily Behavior Chart for Middle School

This is a daily behavior chart that I use for certain students that need that instant feedback. It also helps keep communication open with the parents. It's designed for middle school students that go to different teachers throughout the day. It's designed to provide the students and parents with feedback while being simple for the teacher to fill out quickly at the end of each class. It can also be used for high school students if needed.

Assignment Book or Planner for Organization

This template can be used for students who benefit from a daily/weekly Assignment book or planner or agenda (whichever wording your school uses). In my district we are very technology based. However, as a special education and regular education teacher I have learned there are some students who NEED a written planner to track assignments. There is a title page, which you can decorate to your liking. There is also a parent initial column. Feel free to tweak and add a teacher signature portion

They're working for a goal, and staying accountable with the help of teachers and parents. Personalize the tracker to match the student's schedule, and watch their organization and assignment completion improve!

I created this planner for my 5th grade class to help them keep track of their assignments as well as to help them develop the organization skills that would become increasingly important as they furthered their education. They simply copied down what I wrote on the board each day and I stamped it signifying that what was written down was indeed accurate; a great way to keep parents in the loop as to what was happening in class.

This is a great way to communicate with parents on a weekly basis. Coupled with the Weekly Homework Envelope, this form helps communicate student progress. Student work is sent home once a week and students record grades on this report. Parents are able to see which assignments have not been completed and can ask questions and write comments. I use an online gradebook that parents have access to, but this form is an integral part of how I manage classroom behavior, keep track of academic achieve

A great tool for students to use at home. This one-page document includes a checklist for both mornings and evenings, including such items as: Do I have all of my equipment for extracurricular activities? Do I need to have any notes or forms signed by my parents? This checklist can be laminated so that students may check off items with a dry erase marker. It's great for posting at home by the door! The entire checklist could be covered as a mini-lesson in class, too.

Strong Work Habits Dry Erase Poster (if laminated)

This is a 8.5 x 11" poster that I created for a handful of students who were failing my class, constant procrastinators, disorganized, and so on. I was tired of calling their parents saying the same thing over and over. This is an academic intervention for the students to take home, hang on their fridge or near their homework space, and to use every night. This also works for students whose parents mean well but work long hours and aren't there to reinforce strong work habits. If you laminat

Having Students Take Initiative for their Grades

Here and there I have my students fill one of these out after looking at their grades, so that they can become more responsible. The great thing about this is it allows them and their parents to see what actions they can take in order to get a better grade. Something I really believe is that most students who have bad grades, have that grade because they don't know how to get a better grade. This sheet allows students to own up to why they have a bad grade, while also allowing them to ask themse

Tips for Parents of High School Students

This is a tip sheet we mail out to parents of all of our freshmen before the school year starts. The tip sheet addresses issues such as grades, goals, communication, common pitfalls, and even some don'ts for parents. Since, as a counselor, we see so many of these issues arise during the year, we felt that sending out a tip sheet might help. I am not sure if the sheet did or did not help, but it has been well-received. The file is Word format. It can be easily altered to fit your needs.
