This is a daily behavior chart that I use for certain students that need that instant feedback. It also helps keep communication open with the parents.
It's designed for middle school students that go to different teachers throughout the day. It's designed to provide the students and parents with feedback while being simple for the teacher to fill out quickly at the end of each class. It can also be used for high school students if needed.

Assignment Book or Planner for Organization
This template can be used for students who benefit from a daily/weekly Assignment book or planner or agenda (whichever wording your school uses). In my district we are very technology based. However, as a special education and regular education teacher I have learned there are some students who NEED a written planner to track assignments. There is a title page, which you can decorate to your liking. There is also a parent initial column. Feel free to tweak and add a teacher signature portion

Student Homework Planner
I created this planner for my 5th grade class to help them keep track of their assignments as well as to help them develop the organization skills that would become increasingly important as they furthered their education. They simply copied down what I wrote on the board each day and I stamped it signifying that what was written down was indeed accurate; a great way to keep parents in the loop as to what was happening in class.

Academic Progress Report
This is a great way to communicate with parents on a weekly basis. Coupled with the Weekly Homework Envelope, this form helps communicate student progress. A great tool for students to use at home.
This one-page document includes a checklist for both mornings and evenings, including such items as:

Strong Work Habits Dry Erase Poster (if laminated)
This is a 8.5 x 11" poster that I created for a handful of students who were failing my class, constant procrastinators, disorganized, and so on. I was tired of calling their parents saying the same thing over and over. This is an academic intervention for the students to take home, hang on their fridge or near their homework space, and to use every night. Tips for Parents of High School Students
This is a tip sheet we mail out to parents of all of our freshmen before the school year starts.
The tip sheet addresses issues such as grades, goals, communication, common pitfalls, and even some don'ts for parents. Since, as a counselor, we see so many of these issues arise during the year, we felt that sending out a tip sheet might help. I am not sure if the sheet did or did not help, but it has been well-received.
