I have been tutoring privately for over 25 years and I fine-tune my tutoring policies every school year and summer. This is my latest version. You will have to "make it your own." It is NOT a one size fits all product so, of course, you will have to customize it. I always send the first 2 pages to parents after the initial phone call or email so they understand my background, my services, and Orton-Gillingham. The final 2 pages are used after a schedule has been agreed upon between the parent(s)

We use this in our first grade class to help students understand the consequences of their actions. They "Stop and Reflect" in order to make a better choice the next time.
*I have recently updated the document to include a middle grade version for 3-5 with more grade appropriate wording and formatting!
After it is completed, fold in half to keep private. The student signs it, the teacher signs it, then it goes home for the parent to sign.

Inside Out Note Taker and Key
This note taker is to be paired with the Pixar film, Inside Out. It consists of 22 questions that are intended to keep students focused on the movie and not talking, texting, or sleeping throughout. The last four questions are reflection/critical thinking. On the first day of high school I have my 9th graders fill out a survey about themselves (favorites, etc.) and write a letter to their "Senior-Self" to receive at graduation. This is such a rewarding activity that has been such a joy each year!!!

This assignment is called the Poor Man's Budget. This assignment forces students to work 40 hours a week at minimum wage. They must calculate their gross pay, taxes, net pay, and all of their expenses.
