Great executive function exercise that is functional. A short scenario is given and the client must provide items that need to be packed for each vacation. Help client determine what clothes they would pack during certain types of weather or different types of location. Relate this activity to real life, comparing seasons and the differences between clothing. Also, what items would need to be taken for each trip. If it is hot and sunny, sunscreen would be necessary. This word doc contains multiple passages from famous pieces of literature. Dashes are placed throughout the passages, to remind the student to take a pause and breathe before continuing reading. Especially useful for stuttering and cluttering clients who are unintelligible. Maintaining phonation and over annunciation allows the student to be much more intelligible to a listener.

One forms is more for upper grades and one targeted more for younger (elementary). Sometimes we need more valid teacher input either for an evaluation, upcoming annual meeting, to see if our therapy is generalizing, or purely to satisfy consultation time. That is what these forms are for. They are both editable so you can use the template but focus on your specific goals. This is mostly for SLPs.

I use these pages after any IEP meeting when I need general education teachers and specials teachers to sign that they have seen a child's IEP.
