This is a behavior chart I created that puts homework completion, effort and behavior in class, on-time to class status, and a spot for the student to write assignments, all on one sheet of paper. I used this at the middle school level (there is a spot on the chart for each period for the student who uses it), but elementary teachers could modify it to include the different times of the day. Highly successful tool!
