I designed a digital version of this paper to help my students transition from the Primary Handwriting Paper used to teach kids to write to the traditional wide ruled.
I found that many students - in particular our special needs students- would fill the entire with a big letters and disregard where the middle hump should be. They couldn't envision it on their own - their little minds still need it! And I just couldn't find what I wanted anywhere.

Occupational Therapy Screening Request Form
This is a form school-based occupational therapists can use for screening students. I like to give this form to all of my case managers as well as any general ed teacher who requests it; the person filling it out can check off their concerns like fine motor skills, emotional regulation, difficulty with transitions, etc. There's also a section for the OT to fill out their observations and recommendations after completing the screening.
