I teach science lab at an elementary school, so I see 20 classes of K-4 students per week. Each class has its own behavior plan -- clip chart, token economy, etc. -- and I can't keep up! So I created this behavior report slip to help me. If a student makes a poor choice during his/her time with me, I simply fill out a slip and give it to his/her teacher when she returns to pick her class up after the hour is over.
