This is a great way to receive and keep information from parents that is PAPERLESS and it is editable! I refer back to this form many times throughout the year as needed. Parents fill this form out while my kindergarten students are getting screened. You can also send this out in an email for parents to respond (if you are starting the school year online). I LOVE getting confirmation about dismissal, this makes day 1 go so much smoother! YOU MUST HAVE A GOOGLE DRIVE TO ACCESS AND EDIT THIS FORM!
