It is NOT a one size fits all product so, of course, you will have to customize it. I always send the first 2 pages to parents after the initial phone call or email so they understand my background, my services, and Orton-Gillingham. This is a great way to communicate missing homework to parents. I send this home when students have missed more than 3 days in a month. It contains tips for getting homework done, a line for students to create a "homework plan" and a parent and student signature line. The form is also included in Spanish.

Assignment Book or Planner for Organization
This template can be used for students who benefit from a daily/weekly Assignment book or planner or agenda (whichever wording your school uses). In my district we are very technology based. However, as a special education and regular education teacher I have learned there are some students who NEED a written planner to track assignments. IEP Questionnaires - Editable
This is a fully editable document. Questionnaires for parent, student, general education teacher, music teacher, PE teacher, ELL teacher, and parent. These are used for developing an annual, innitial, or triennial IEP. They can also be used for developing a 504 plan. Using these forms helps make an IEP more appropriate as it incorporates input about the student from every environment. It also helps IEP meetings run more quickly and efficient as everyone's input has already been gathered.
