This simple exit ticket enable students to quickly identify whether or not they "got" the day's lesson, "sort of got it", or if they still need help. Teachers can easily flip through the exit tickets and analyze formative assessment data without having to read through piles of paperwork. Teachers can then modify the lesson for the next day based on the results, and address student misconceptions, if needed. This tool is easy to use and helpful as a formative assessment strategy.

This file is in Microsoft Excel because I am able to edit the document far easier compared to a Microsoft Word document.
