Behavior Tracker (Student Self-Monitoring Report) - EBD Classroom
This self-monitoring sheet is something I use to track behavior in my 4th-5th grade self-contained classroom for students with emotional/behavioral challenges. "I Get It" Exit Ticket
This simple exit ticket enable students to quickly identify whether or not they "got" the day's lesson, "sort of got it", or if they still need help. Teachers can easily flip through the exit tickets and analyze formative assessment data without having to read through piles of paperwork. Teachers can then modify the lesson for the next day based on the results, and address student misconceptions, if needed. This tool is easy to use and helpful as a formative assessment strategy.

I felt like this was a great way to keep students accountable for their work. Also, I had some students write down the correct answers in the boxes and use it as a study guide for a test/quiz.

Teaches the student how to use the round numbers formula by rounding to whole, tenths, hundredths, thousands and negative whole. In the process the student will review writing formula's, inserting columns, formatting a sheet and other excel functions.

This is a spreadsheet in excel that you can use to come up with the amount of ingredients in a recipes. All you need to do is put your own recipe and how much you want to multiply it by. This can be used for the Life skills class or at home. I hope this helps everyone.

Facebook Student Introduction Page
I use this Facebook Page template at the beginning of every year for my students to complete. I ended up having to create my own after many failed attempts to upload from Pinterest. This file is in Microsoft Excel because I am able to edit the document far easier compared to a Microsoft Word document.
Not only does it ask what they are interested in, it also gives them the opportunity to tell me how they like to learn, ask me any questions, and tell me a bit about their friends and background.
