Goal sheet for 22/23 school year

I use this goal sheet with upper elementary during the first week of school. I like for them to evaluate and reflect on the things they need to do to reach their goals for the year. I collect it at the end of class and give it back to them during the last month of school (preferably right before state testing) so they can see the progress they have made.
