I used this goal sheet for individualized reading goals. Each student got their own mountain (differentiated) and glued it into their agenda / planner. I used it for reading goals, but you can use it pretty much for everything including math facts or behavior management. This way they can monitor their own progress and even lower students can see that they are making progress. Also helps teach percentages or you could change it to fractions. :-)
