I struggled with my students being accountable during independent reading time. I wanted to make sure that they were really focusing and thinking while they were reading. Having them fill this out at the end of each reading session has really helped. While reading they are paying much closer attention and placing sticky notes at parts where they might want to do a response. When about 3 minutes are left of Independent reading time, the students write in a quick response.
