I love this so much! Sometimes parents need some help with narrowing down what they want to say in regards to their kid's IEP. I wish this was available in Spanish!
Excellent resource to help parents think about specific topics to cover in the parental input section of the IEP for their child's unique and individualized needs.
Description
It can be difficult to gather meaningful yet concise information about parental concerns when writing an IEP. Parents don't know exactly what kind of information you're looking for; accurate documentation of a conversation is a hassle; not to mention, no one has time for a formal interview. This survey can be sent home and returned for easy collection of short, informative, multiple choice statements as well as space for additional information in written form. I like to send mine home with the Notice of Proposed Meeting to give the parents plenty of time to respond thoughtfully.
