Student Evaluation for Teachers
This is a one page sheet with twenty different statements for students to rate about their teacher. They put a check under one of four different rating options: Always, Most of the Time, Some of the Time, Never. There is room on the bottom for the student to write specific comments as well.
I use this every year. It is wonderful (and a bit brave) to view the students' honest feelings toward me. It is totally anonymous. Great reflective tool for teachers.
