This is a layout I use for my lesson plans. My school has the Houghton Mifflin-Harcourt "Journeys" curriculum. Every day follows the same process, so those are inprinted in the lesson plan already. There is also a place to add the standards that are taught each week.

Behavior Chart
I use this chart for recording student behavior throughout the day. After 2 warnings they will use a part of their recess time to reflect upon their choices. After their time is finished I conference with them and ask them how they could have chosen differently. Many times students do not remember the details of the incident. Substitute Evaluation Form
Sick of getting terrible substitutes? Using this form allows you to document the various substitutes that have been in your classroom and what they have or have not accomplished. This form will help you keep track and remember and hopefully get that terrible substitute banned from your classroom.

Pirate Behavior - Behavior Log
This is a chart to help the teacher keep track of class behavior weekly. There is a column for awarding points for good behavior (can be used with whatever PBIS system you already have). There is a column for recording "clips down" (if you use the clip chart) or behavior marks. The last column allows you to record the color the student is on at the end of the day.

Behavior Records
I am using these to keep track of my students' daily behavior. I use behavior cards (green, yellow, blue, and red.) At the end of each day, I will highlight the date with the color of the students' behavior cards. This will be great data for parent teacher conferences.

I made this chart to keep track of my students' progress, behavior, parent communication, etc. I print out a bunch of these and place them in a three ring binder per child. Anything I need to document go under the child. This helpe me to keep track of what has gone on all year long with my students.
