This is a weekly 'mini' book report my students fill out to help demonstrate that they are comprehending what they are reading. My students have about 30 minutes of silent reading time a day, this helps get a quick check that they are actually invested in the book they are reading. As the weeks through our reading curriculum progress I change the skills on the organizer and add new ones.
This is an individual assignments that students receive on Monday and turn in on Fridays.
